A Server Error in '/' Application error shows up when accessing the SolarWinds Platform Web Console.
Upon reviewing the services using the SolarWinds Platform Service Manager, you'll notice that some of them have stopped; SolarWinds Collector, Recommendations, Module Engine, etc. However this can also be caused by a stopped Application Pool in IIS.
C:\ProgramData\SolarWinds\Logs\Recommendations\Recommendations.Services.log will show SQL-account logon errors below:
[SolarWindsRecommendations.OnStart] WARN SolarWinds.Orion.Swis.PubSub.MessageBus.MessageBusTopologyProvider - Could not load updated message bus host list from database.
System.Data.SqlClient.SqlException (0x80131904): Login failed for user 'SolarwindsOrionDatabaseUser'. Reason: The password of the account has expired.

Cause
SQL account used by the SolarWinds Platform is unable to logon to the database because its password is expired.
Resolution
- Contact your DBA to remove 'Enforce password expiration' on the SQL account used by the SolarWinds Platform.
- How to Start, Stop, or Restart SolarWinds Services with the SolarWinds Service Manager in the SolarWinds Platform
- Reboot the SolarWinds Platform Server.
Note: Also check the SQL database if its accessible and there are enough disk space to operate.
