AppInsight for Exchange is producing "Performance counter updating error" event logs on the Exchange server

Performance counter updating error. Counter name is Connection Leak, category name is MSExchangeRemotePowershell. Optional code: 3. Exception: The exception thrown is : System.InvalidOperationException: The requested Performance Counter is not a custom counter, it has to be initialized as ReadOnly.

Cause
This issue occurs after the initial set-up of Exchange Polling or occurs after cumulative updates have been applied to the Exchange.
The root cause of the issue is that the Performance Counters cannot be loaded.
Resolution
To resolve this issue please refer to the Microsoft KB - "Performance counter updating error" after you install an Exchange Server 2013 cumulative update (© 2018 Microsoft, available at https://support.microsoft.com, obtained on October 15, 2018).
