If you deleted or lost the SQL Server user that DPA uses for monitoring a SQL Server database instance, follow these steps to recreate the user with proper rights and privileges:

  1. Connect as a SYSADMIN user to the monitored database.
  2. Replace username and password in the script below with the credentials that were deleted.
  3. Run the script:
    --Create DPA login

    CREATE LOGIN [username] WITH
      PASSWORD=N'password',
      DEFAULT_DATABASE=[master],
      DEFAULT_LANGUAGE=[us_english],
      CHECK_EXPIRATION=OFF, CHECK_POLICY=OFF
    GO

    --Give SYSADMIN rights to the DPA user
    EXEC sys.sp_addsrvrolemember @loginame = N'username', @rolename = N'sysadmin'
    GO
